Ultimately, you are responsible for the services you receive from us. However, we are willing to bill your primary and secondary insurance carrier for you. Please provide us with accurate information.
For accurate billing, please bring the following items with you:
- Photo identification
- Insurance card
- Signed order or prescription from your doctor's office with diagnosis
- Form of payment for copayment, co-insurance, or unmet deductible, if applicable
